ABOUT ALTERNATIVE HOME HEALTH CARE

OUR MISSION

At Alternative Home Health Care, our mission is to work in active partnership with all community providers and caregivers to ensure that all our patients receive the highest quality of care and support needed in helping them stay independent in the place they call home.

We are committed to always respecting our patient's individual rights and actively support, promote, and encourage their participation in their health care choices.

OUR VALUES

  • Respect and value one another as unique individuals and practice open and honest communication.
  • To look beyond the obvious solutions and accept accountability for delivering positive outcomes for our patients.
  • To always do the “right thing” and advocate for our patients.
  • Care for our patients with compassion and openness while striving to exceed their expectations.

OUR GOALS

We strive to build strong relationships with our patients, staff, referral sources and to give back to our community whenever opportunity is there to do so. We seek to minimize inpatient hospitalizations and ease the transition from acute or rehab settings back to the patient’s home and community. We strive to be recognized by the community for our passion and commitment to serve, help, and improve the quality of life and well being of others. Also, we will foster an empowering work environment for our staff that promotes professional growth, continuous learning, and a team atmosphere and approach. We seek to inspire, lift and provide the tools and skills to our patients and staff to reach their goals.
